DOT Verifications
In accordance with regulations of the Department of Transportation (49 CFR Part 40), we will contact the driver’s former employer and request information concerning the named driver’s past drug and alcohol test results, including refusals to be tested. Questions asked are:
• Did the employee have alcohol test results of 0.04 or higher?
• Did the employee have verified positive drug test results?
• Did the employee refuse to submit to a DOT required drug and/or alcohol test?
• Did the employee have other violations of DOT agency drug and alcohol testing
regulations?
• Did a previous employer report a drug and alcohol rule violation to you?
• If the answer was "Yes" to any of the above items, did the employee complete the
return-to-duty process?
